This package supports palliative care services to effectively use the PCOC audit tools to improve compliance of clinical assessment and identify areas for improvement.

Quality Improvement Audit Tools:

NEW Case Review Templates:

  • Deaths outside the terminal phase - to improve consistency in the correct identification and recording of the terminal phase.
  • Severe family/carer problems - to improve the consistency of the assessment and management of severe scores on the family/carer domain of the PCPSS.
  • Severe pain - to improve the consistency of the assessment and management of severe pain scores on the PCPSS and/or SAS.
  • Patients in unstable phase of four days - to identify reasons why Patients remain in the Unstable Phase for more than 3 days, and to ensure the Phase is ended appropriately.
