Central to the PCOC program is a framework and protocol for routine clinical assessment and response. By embedding the framework into routine clinical practice, clinicians have access to consistent information to plan and deliver care.
Whilst the program initially developed in the specialist palliative care sector, it has now expanded its scope into all setting of care where patients receive palliative and end of life care.

Assessment forms

• Assessments are routinely completed as a suite at each assessment occasion.

Profile data collection forms 

The profile data collection is used to capture a single point of assessment of palliative and end-life-care in any setting.

A profile of palliative care captures a palliative care assessment that is:

• clinically significant in nature;
• intended as a one-off event;
• may be followed by subsequent assessments and or episodes of palliative care;
• may be delivered in any setting (for example, hospital, home, RACF, clinic);
• may be delivered via any mode (for example, in-person, telehealth or telephone).

Select the form that matches your service type:

1. Palliative care services (adult or paediatric) Patients referred for one-off palliative care assessment 
2. Non-palliative care services (primary care, acute care, aged care) Patients identified for palliative assessment
