The purpose of this toolkit is to support palliative care services and clinicians to implement, embed and sustain the Palliative Care Outcomes Collaboration (PCOC) program within the routine practice of their service.

Package to Implement, Embed and Sustain PCOC

This package provides support for services to implement, embed and sustain the PCOC program within routine practice. It contains the Guide to Implement, Embed & Sustain the PCOC program, Assessment Protocol, Guides for assessment, Clinical Manual, Embedding & Sustaining PCOC Bi-annual self-assessment and a tool to map PCOC to the National Quality and Health Service Standards (NQHS).

Assessment Package

This package includes the PCOC Assessment Protocol and Clinical Assessment Tools, Multilingual Symptom Assessment Scale (SAS), PCOC Assessment Forms and the Orientation to Assessment Tools.

Mapping to National Standards

This resource has been endorsed by the Australian Commission on Safety and Quality in Health Care and will support the accreditation efforts of the palliative care services that are participating in PCOC.

Quality Improvement Audit Package

The audit package contains tools to support palliative care services to monitor and evaluate clinical assessment practice and identify areas for improvement.
